Reports from Educational Psychologist (not older than 12 months) which must provide details of the standard assessments used as per PD026 Annexure A
A report from a relevant healthcare practitioner.
There must be a relationship between the physical impairment and specialist physician.
Report must include the diagnosis, treatment, management, support and justification of assessment accommodation/s.
Evidence indication hospitalisation.
